A number of Primary Care Clinics are held at various locations across the City and County. These are for people referred by Primary Care Teams.

Below are the relevant referral forms, and Information for Primary Care Professionals. This includes information on the Nutrition and Dietetic Service available to Primary Care, including contact details, referral procedure and referral criteria.

Referral Form - Adults

For use by health professionals to refer adult patients for an outpatient appointment in primary care. If your patient is housebound, please refer to our referral guidance for housebound patients below.

Patient Client Sessions

Dietitians across Leicestershire and Rutland provide education sessions for patients and the public. This can often be initiated by the Primary Health Care Team (e.g. group education sessions for people with diabetes) or can be at the request of support and voluntary groups. Depending on the organisation that requests the talk, there can sometimes be a charge.
