Pre-Surgery Bariatric
Referrals for the Community Pre-Bariatric Assessment should be made via the PRISM Weight Management Tier 3 (Adults) referral on the Home HUB page.
Community Primary Care Clinics
Paediatric weight management referrals should be made via SystmOne electronic referrals where possible, or via PRISM referrals for EMIS practices. We are also able to accept SystmOne task referrals if sufficient information is provided from Community Services/other Health Professionals.
