A number of Primary Care Clinics are held at various locations across the City and County.  These are for people referred by Primary Care Health Professionals/Teams. 

We accept referrals for Adults/Paediatrics/LASS/Specialist Obesity Pre-bariatric/Domiciliary (tel contact)
Referrals to our Service should be made via:
  • SystemOne - PRISM and E-refer for SystmOne users
  • PRISM - for non SystmOne users including EMIS practices (available on the HUB home page) and sent to
  • Electronic referrals via or E-refer for Community staff with limited access to SystmOne
  • If you do not have electronic access, please contact us on 0116 2227170 for further support

Adult Nutritional Screening Tool for use in the Community

To be used by GP's, Practice Nurses, District Nurses, Care Home staff.

Referrals for patients in Care Homes

These should be made via the patients GP as a Domiciliary referral as above - clearly showing that LMSG guidelines have been followed and food first advice implemented.

Patient Client Sessions

Dietitians across Leicestershire and Rutland provide education sessions for patients and the public.  This can often be initiated by the Primary Health Care Team (e.g. group education sessions for people with diabetes) or can be at the request of support and voluntary groups.  Depending on the organisation that requests the talk, there can sometimes be a charge.
