Outpatients

A number of Primary Care Clinics are held at various locations across the City and County.  These are for people referred by Primary Care Health Professionals/Teams. 

 
We accept referrals for Adults/Paediatrics/HIV care/Specialist Obesity Pre-bariatric/Domiciliary (tel contact)
 
Referrals to our Service should be made via:
  • SystemOne - PRISM and E-refer via tasks for SystmOne users
  • PRISM - for non SystmOne users including EMIS practices (available on the HUB home page) and sent to LLR.nutritiondietetics@nhs.net
  • Electronic referrals via LLR.nutritiondietetics@nhs.net or E-refer tasks for Community staff with limited access to SystmOne
  • If you do not have electronic access, please contact us on 0116 222 7170 for further support


Referral Criteria - Adults

Housebound patients (Domicilary telephone contact) referrals should be made as above using the PRISM Domiciliary forms and by following guidelines:
 
 

Adult Nutritional Screening Tool for use in the Community

To be used by Practice Nurses and District Nurses where scales are not available to weigh patients.
 
 

Referrals for patients in Care Homes

These should be made via the patients GP as a Domiciliary referral via the methods as above - clearly showing that food first advice has been implemented and LMSG guidelines have been followed.
There is a different referral route for care homes currently serviced by the Enhanced Medicines Optimisation Team (Prescribing), those care homes will have been contacted with details. If unsure, please contact 0116 222 7170
