Nursing and Residential Care Homes

Dietitians work with nursing and residential homes to provide support to staff on screening for undernutrition and managing residents are risk of undernutrition.  This can involve providing training and resource materials for care home staff.  It also involves working with GPs and care home staff to ensure that prescribed nutritional supplements are being used appropriately.

Dietetic Referral form for Care Homes

To be used by care home staff to make a referral to a dietitian.  Once completed this should be sent to the patient's GP, along with recent 3 day food record charts, so the GP can make the referral.
