Nursing and Residential Care Homes

 
Dietitians work with nursing and residential homes to provide support to staff on screening for malnutrition and managing residents who are at risk of malnutrition.  This can involve providing training and resource materials for care home staff.  It also involves working with GPs and care home staff to ensure that prescribed nutritional supplements (ONS) are being used appropriately.

Dietetic Referrals for patients in care homes

Referrals from care home staff must now be made by the patients GP via SystmOne or PRISM to LLR.nutritiondietetics@nhs.net.
MUST screening tool should be completed together with adherance to the LLRAPC guidelines
Community staff can refer care home patients via E-refer tasks where they have access to SystmOne.
 
Those care homes under the dedicated Enhanced Prescribing/Medicines Optimisation Teams have a separate referral process. Please contact or Headquarters for details. 0116 222 7170
